Healthworks Cornerstone

Target Audience State and local agency personnel working in the Family Case Management program for DCFS Wards.
Course Length 2 days
Class Times Day 1: 9:00 am - 4:30 pm
Day 2: 9:00 am - 12:15 pm
Description Purpose: To provide software and appropriate policy and procedural instruction for the Healthworks Program to ensure that local service providers have the knowledge and skills required to meet program standards.

This training will provide a “hands-on” approach to the Healthworks/DCFS functions within the Cornerstone software system. The course includes: Overview of the Healthworks Program, Documentation of DCFS Ward Data, Medical Care Plan Development, Assessment and Case Notes.

Performance Objectives Upon completion of the course, participants will be able to:
  • Demonstrate proficiency in enrolling a new client on the Cornerstone system
  • Effectively complete the processes required to enroll a client into the Healthworks Program
  • Complete Family Case Management activities as they relate to DCFS Wards and the Healthworks program (assessments, medical information, and case notes)
  • Complete a Health Works Care Plan, placing emphasis on medical issues
