This in-person training is designed to give Maternal Child Health Nurses an overview of their role in the IDHS prevention-focused MCH programs to prioritize the engagement between the dyad and their medical home. The course will explore the nurses’ role to support health literacy when engaging with birthing families in the community public health setting.  However\, please note that this course is not an ESPDT course\, as the DHS programs are not intended to be a replacement for assessments done by the medical home. \n*Registration for this course closes one week prior to the training date. \nPlanner: Renee S. The classroom training is designed to address the needs of new case managers and cover each step of the Case Management process: client identification or outreach; assessment; service planning and resource identification; linking clients to services and coordination; monitoring services; advocacy and evaluation.
