- WIC Material Order Form
- The order form must be filled in electronically and should be submitted to DHS.WICMaterials@illinois.gov for processing. Hand written and faxed requests will not be processed. Currently order are taking 6 - 8 weeks to be fulfilled. Agencies should keep inventory of handouts and make bulk orders quarterly to avoid stock being depleted.
