Introduction to Medically Prescribed Formula

Course Description:
To provide the most current information to new Competent Professional Authority (CPA) staff working in the Illinois WIC program on the fundamentals of medically prescribed formulas

Performance Objectives

Upon completion of the course, participants will be able to:

  • Identify the Illinois WIC Program Formulary and medically prescribed formula product characteristics.
  • State the WIC Program policy and procedure requirements on the assessment, education, and referrals regarding medically prescribed formulas
  • Demonstrate understanding of the WIC MIS for food package assignment, documentation on the formula and medical nutritional prescriptions form, and benefit issuance of medically prescribed formulas.

Special Instructions

By registering for this training, I certify I have completed and agree to the following:

  • CPA Orientation Checklist, all items apart from Illinois WIC State Trainings
  • WIC clinic experience as a CPA for a minimum of 60 days; 90 days is preferred
  • Access to equipment and a training environment conducive to remote-based (online) training
  • Completed the ‘CPA Competencies for Issuing Formulas SSM’ as part of orientation checklist; reviewed SSM with supervisor and familiar with Illinois WIC Contract formulas and issuance
  • Attendance for the entirety of the training and participation throughout the training are required to receive a certificate of completion and continuing education credit. Trainees may be asked to participate by speaking, utilizing the chat function, submitting activities, or other modes of interaction.
  • Please note: If fewer than 5 trainees register for this training, it may be canceled and registrants will need to register for the next available date.

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education through the joint providership of Southern Illinois University School of Medicine and Urban League.

CREDIT STATEMENTS

SIU School of Medicine is preapproved as a continuing nursing education provider pursuant to Section 1300.130, subsection c), 1), B) and P) of the Illinois Department of Financial and Professional Regulation Nurse Practice Act.  Nurses may receive a maximum of 3 contact hours for completing this activity.

SIU School of Medicine is a licensed provider for continuing education for dietitians/nutrition counselors (license # 199.000121). This program offers a maximum of 3 CE hours.

All other attendees will receive a Certificate of Participation.
